Non Registered Plans: Descriptions and functions of the following forms:

Form Purpose
T5 Reports interest and dividend Income.
  • Not issued if amount is less than $50.00
  • Normally issued in January
T3 Issuer Purpose
BMO InvestorLine
  • Reports distribution for Trust Units, REITS, XIU's
  • Normally issued mid-March
Mutual Fund Company
  • Distributions on the Mutual Fund by the Fund Manager
  • Normally issued late March
T5013 Reports taxable income or loss from Limited Partnership Units.
  • Normally mailed out mid-late March

Registered Plans: Commonly used Tax Forms for Registered Plans:

Form Purpose
T2033 Used to retain the tax-sheltered status of an RRSP/RRIF transfer from one Investment Firm to another.
TD2 Code/form that is used to signify a Retiring Allowance Transfer or severance payment from a client's employer to an RRSP.
  • The transfer is coded as a "TFJ" in order to have the 60(j) receipt issued which is used to offset a T4A slip from the employer.
  • The actual form is seldom used.
T4 RRSP Reports amount of withdrawals and any withholding taxes from an RRSP.
T4 RRIF Reports income received from an RRIF account.
  • Includes minimum payments, any additional amounts and tax withheld (if applicable).
T2151 Form used to transfer lump sum payments in from a Registered Pension Plan (RPP) or Deferred Profit Sharing Plan (DPSP).
T2030 Form used to transfer funds from a RRIF back to an RRSP or from an LIF back to LIRA (as long as the client has not reached the age of 69 and can no longer operate an RRSP/LIRA).
T1036 Form used to withdraw funds from the First Time Home Buyer's Plan.
RC96 Form used to initiate the withdrawal of funds from the Life Long Learning Plan.
  • Proof of enrolment should be included.
T2220 Form used to transfer RRSP Assets between spouses upon Marriage breakdown/divorce.
  • An original or Notarized copy of the "Separation Agreement" should also be provided.
